I would say a sexy photo of you! My boyfriend loves that. Maybe you can have it done professionally, go to a studio and have them take pics of you in his football jersey or something, guys love that!! That way he won't get too big headed over himself and then he'll know that he's not the only hot person in the relationship! hehe

I would take him to a romanic dinner followed by sitting and looking at the city lights somewhere romatic on a blanket enjoying a drink or two and talking about what ever comes to mind and then give him something that he has been wanting at that time like a nice jacket or cd or out fit or what ever he enjoys.
