Go to your local post office. Most will have the forms and be able to take your picture and do everything that is involved for you. The cost is usually around $90 for standard passport, double that if you need to expedite it.

Click on the WWW at passport.unitedstates.org
You will find the forms, instructions and fee.
The photos can be obtained at many drugstores, such as Walgreens. Maybe even WalMart.

check with the state department webpage, that'll tell you and have forms you can download, and give you info about which post offices take passport applications.
