Yes they could, as the five precepts (which you are referring to) are not a rigid set of rules, they are guidelines so that the Buddhist has the right mind to attain enlightenment. They use reason to determine what course of action to take when conflicting circumstances arise, and so would be able to break the precept "Do not take intoxicants" if they were in pain. This would not result in bad karma, as there is no unskilful action with immoral intention- the intention, to ease pain, is positive and would lead to positive karma.

It should be pointed out that all kinds of diseases are treated with medicine or with many procedures of medical science. Some treatments use our own intentions and will power, for example, psychotherapy or the practice of meditation. In such treatments, the patient must play a part in helping himself, not simply depending on medicine. These treatments demonstrate that the mind can look after itself and have an effect on the treatment of physical disease. In this way, if a patient receiving treatment is able to understand that his sickness is physical and doesn't allow it to affect his mental health, staying calm and cheerful, then that his sickness will inevitably improved and be cured more quickly. But if a patient reverts to being low-spirited, depressed or self-piteous then the sickness will be more difficult and take longer to treat. Therefore, staying calm, clear-headed and cheerful at all times is something that can protect us from disease.
medicines can be considered as those things that are specifically for illness.
in fact many buddhists provide medical and nursing care with volunteer input.

I agree with S*x-kitten. The precepts are guidelines and no Buddhist would want someone to suffer needlessly if there was no alternative. A Buddhist would probably prefer to try other methods of pain relief over drugs and they have many such as meditation techniques, chanting, herbal remedies, acupuncture, etc...
If someone's pain is out of control and they can't manage it via other techniques than I would say they already are not having a clear mind due to the pain.

it helps to have a clear mind but if the pain too much then you will not have a clear mind beacuse of the pain! i would think it would depend on what you motivation is for having it, if its for the fun of it then yes bad karma, but if its to help with pain then no problems. this is such a difficult question to answer without going in to it deeply!! and then i would be here all night!

they believe that it is bad karma that causes the pain, thus would prefer to take the pain to bring their karma back to an even balance
