I have my birth certificate handy and there isn't a place for the recording of religion. All it has are:
1. Name and Surname
2. When and where born
3. Sex
4. Name, Surname and rank or profession of father, Name and maiden surname of mother. Date and place of marraige
5. Signature and qualification of informant, and residence, if out of the house in which the birth occurred
6. When and where registered and signature of registrar
And I'm not aware that medical records take account of religion unless there are medical implications like Jehovah's Witnesses refusing blood transfusions (or mental health issues), for example. I don't think there's anything to change on these things.
You will have been Christened and the church will forever claim possesion your soul, albeit dedicated without your permission. There's nothing you can do about that but know better.

I am also a Buddhist.
If you want to change your birth cert. to validate your decision in your own mind I say you should do it and be glad.
But I would like to warn you that it is not wise to forget who you are and where you and your family came from, and it could be a good reminder of who you are to keep the religion on your birth cert. as it is.
Oh, I have no idea if you can change your birth cert. or how you change it. So no, I did not answer your question.

I dont think it is on the birth cert??
as far as changing any details on your medical records, have a word with your doctor

I changed my religion too, but I don't think it is written in any of my personal documents..I have never checked for sure though. besides, it does not matter what is written there. Only your faith matters...

Doesn't matter what's written on documents. Matters only what path your on. If you want to change who looks after you spiritually in hospital, let them know you'd rather have a monk, if one is available, than a priest. Buddhism is a wonder peaceful path. Hope you find what you're looking for.

tell your doctor and the office of personal documents (i don't know what the hell it's called - try the county clerk of your birth county) to change 'em. i doubt they'd hold you to the birth certificate, anyhow.
