"Brutally honest" means being totally honest and telling the truth without diluting it in any manner even though the truth is hurtful/ painful to the person who is being told the truth or with whom you are being honest.

You may also say brutally frank.
Brutal means very hurtful or cruel.Honesty is being truthful.
Brutally honest means stating the whole truth even if it's cruel or very hurtful. Sometimes, it is good but most of the time, not. If you want somebody to change for the better, tell him or her the truth even if it means stating all the negative traits she or he has. This is more of a constructive criticism. However, if you just want to hurt a person, I suggest do it another way.
For medical purposes, doctors often have a hard time explaining things.They in the end opt to be brutally frank.
Let's just say, being brutally honest is hitting the bull's eye,getting directly to the truth of the matter.

Well, it dependds on where you put the emphasis.
You could be BRUTALLY honest--meaning you REALLY don't mind being honest even if it hurts other people
Then there's just brutally HONEST-- meaning you're put in the situation(and thus forced) to tell the truth even if it means hurting some one.

Brutally honest means that someone will give an opinion with no holds. Meaning, to tell the truth, to tell it like it is! To say something with open opinion. To be brutally truthful. To interject an opinion without regard to response.

it means that you have to tell sombody who you care about somthing that would probably upset them very much, the thing is if you dont tell them and they hear it from sombody else it would hurt more, so you must take a chace and be "brutally honest"
