There's a couple things it could be. The first one is that possibly something in his food is making his stool hard, which hurts him to poop. Second if could be constipation, which is my more likely guess. This isn't pleasant, but check the stool for blood traces, or anything unusual. Also look at his rear, see if it looks enflamed or has blood stains. You should contact a vet though, because the constipation could be a problem of dehydration. And the hard poops guess could be a sign of improper nutrition (not your fault). Ask the vet if you can bring in a sample stool, so they can run some tests. They'll probably want to see your cat too, but ask for just a stool testing first. (cheaper w/out the cat there) oh! Another thought came to mind, it could be a case of worms, which will hurt the urinary tract and intestines. That could be causing him pain as well. For now, do what you can to keep the box as clean as possible. As soon as u know he's went, even just a pee, get it outta there. Eliminate bacteria. Hope I've been of some help,Good luck !!
ps: i just LOVE the response of 'the cat is probably confused of whats happening.. that just killed me!

Maybe it hurts? Does he avoid the litter box? That could be a sign of him associating pain with the litter box and trying not to feel the pain so going somewhere else.

Yeah I would take him/her to the vet because that usually means something isnt right. I have 2 cats and they dont do that so its not common. Like someone already said it could be constipated or have bowel problems.

Maybe it hurts him/her. I would take him to the vet so he/she can get examined. Maybe the poop is too large for him/her, maybe change the food. But I would have it checked out. It is cute though.

I would worry that he may be in pain when using the bathroom. I would take him to the vet for a checkup just to make sure.

Probably just a reation to the sensation of relief. If it isn't a cry I would not worry about it.

He/She probably does not know exactly what is happening plus it might feel weird for the cat.
