You might ask the doc if he can apply it to the mucus memebranes of his mouth, although this seems a less pleasant alternative. You might also ask if there is a pill that he can dissolve under his tongue. The problem with taking this drug by ingesting it into the stomach is that it would be intactivated by stomach acid and the liver before it had the chance to work.

What are his reactions to the nasal spray? Is it just unpleasant for him, or does he get seriously and violently ill?

This disease is no laughing matter and he must learn to be compliant with his medications, the alternative is pretty grim. He and his doctor (endocrinologist or nephrologist, I hope) should gave good communication and work together to find a way that he can properly take his medications. This is hardly the place to be getting advice about this serious a situation.
