Babbo is far and away the best Italian restaurant I've ever visited. I didn't even like Italian food much before eating there, but now I count myself among the converted =) Eating there requires making a reservation about a month in advance as well as having very deep pockets, but it's great for a special occasion.

I saw numerous people suggest Babbo and Il Mulino. They are exceptional dining choices. I will now try to give you a few that were not mentioned. I prefer Dominick's in the Bronx off of Arthur Ave. I enjoyed Piccolo Angelo's in Manhattan. It is on the west side, I believe near fourteenth. I have heard rave reviews on Genoa and Rao's but have never personally dined at either establishment. Rao's is near impossible to get reservations for.

Po, Mario Batali's old joint. In East Village - great for small atmoshpere and big on taste. Only takes cash or Amex though.
But for the best loud Italian food, go to Domenic's in the Bronx (arthur ave). No menu , no (actual) bill, no questions
its great! (and family style, next to strangers in a place that looks like an old basement with wood paneling - cant beat it!)
