You should eat the way you feel satisfied provided you do not draw attention of others by making noise while eating or you will be watching at every one to find out whether they are watching at you. If it is a buffet, take food on your plate in reasonable decent quantity and sit in a corner and quietly enjoy your food without watching at others. I feel, the problem is more with yourself than with others. You are too conscious of the fact that you are eating in a five star hotel. The waiter had no business to be stiff at you. You are not feeling comfortable with yourself in a new environment with all the predilections you have with regard to a five-star hotel.
Be normal and be assertive on your demands from the Hotel staff including the waiter since you are paying for all these establishment extras, besides the food.

Relax, be comfortable, enjoy the experience, not everyone is able to go to such a place.
Silverware from outside in.
If anyone is staring and sniggering, their manners are atrocious and should be ignored. They are showing a total lack of class and their manners are worse than anything you could possibly do by accident.
Eat food in the order it is served.
Never gulp liquids. This is not only etiquette. You don't want to risk choking on it.
That waiter should be fired.
I, personally, have dined in five star hotels and ignored most of the silverware and eaten very elegantly in the Indian style, with my fingers. I don't believe European style is better manners or more civilized than Indian style. Just my opinion.
btw, the funniest elegant dining scene ever is in 'Pretty Woman,' starring Julia Roberts and Richard Geer.

Start at the outside with the silverware and work your way in. If you are in a five star dining room, your waiter should be attentive enough to remove the pieces you don't need. Fish and meat should not be served at the same time, unless you are offered a choice. In that case, choose one and eat it, and don't take any of the other.
You can't gulp down wine or water in a five star restaurant, in a one star bar and grill, or at home. You are supposed to DRINK or SIP liquids, not gulp them down. Unless you've just hiked across the Sahara desert.
If you are eating in a five star restaurant, your table companions and other diners should have the good manners not to stare and not to snigger. Just watch what other people do and follow them.
Most importantly, relax and enjoy the meal. In order to rate five stars, the restaurant should be superb and should serve items you will not be able to readily get elsewhere.

Just stick to the manners and etiquettes that dad & mom told you when were a kid. Things like keep your mouth closed while chewing, do not talk while eating, do not make obnoxious sounds while eating, gulping water looks crude and the best way to drink water is in sips not in gulps(water might enter your wind pipe). That'all...
Enjoy ur food and drink.
Forget the waiter ignore him, if it was to embarassing, should have complained to the manager, after all you are paying thro your nose for such a expensive meal and drink + taxes + tips...

With fine dining, there is one simple rule you could observe, you start with the utensils from outside and work yourself to the one closest to your plate. Usually, if say, you don't order salad, the waiter removes the salad fork for you automatically and leaves the soup spoon. If you don't know what to do, wait for your companion to pick up the right utensil and do the same.
Also, buy a book on table manners--it may help you familiarize with different plate settings and how to act. Once you are informed, you'll be less self conscious and it will come to you naturally.

Bertrand Russel in his book Mortals & Others tells that one should not react to events with the notion that what his neigbour will think of him.You have to think yourself whatever it may be.
Particularly the 5 star hotels want to sell mineral water though they are required to provide the best water from themselves as they are star categories. Mineral water itself must not be supplied and billed in these star hotels.
If they are not answering to your calls, you have to annoy them much. When I found that the chicken was full of bones, the waiter and the supervisory were not giving proper answers and I called the Hotel General Manager who gave the reply that" here and there " some faults will be there. I paid the bill by mutilating the currencies and told him here and there things will be like this.
Dont be afraid of these systems and humans. One human should not be afraid of other human being

No waiter has any right to sneer at you. Asking for water is perfectly acceptable, in any restaurant.
As to the silverware, just start on the outside and work your way in with each succeeding dish. Let the waiter remove the silverware with the plates.
Remember: the waiter is there to serve YOU, not the other way around!

Just use cutlerly from the outside in and forget everything else. Waiters are stiff because they earn a pittance and have to serve people who are younger and richer than them. Be kind to their plight. No one with any class would snigger at anyone... ignore it or stop going to bad hotels.
