Regardless of what you converted to, the Church will always welcome you back home.

I was a dedicated atheist - hated everything that God was/is. Now I am a very happy, devout Catholic.

Trust me when I say - you will always be welcome.

Peace be with you.

Depends
What do you mean by convert?
Did you sign any paper works rejecting Christianity?
Did you publicly proclaim a rejection by written word or speech of Christianity?
What Jewish religious rituals did you participate in?
Convert means certain actions. To clarify your position with Catholicism I would suggest you first recall the steps that you took to your understanding of conversion to Judaism. With recall what happened?
After you have reached this position and clarified you thoughts and emotions I would approach your parish priest. Phone ahead and schedule an opportunity to discuss this matter with him or his pastoral associate.. Peace be with you--Shalom
