Here is a list of the phrases you asked for..I added a few more to help, and gave pronunciation....
English is very widely spoken especially amongst the young people, and in Prague...
So you won't have a problem communicating...
But it is always nice to know a few phrases in Czech.... And it makes the Czechs happy to see a tourist trying with the language :-)))

I think the most useful and easiest to remember are...

Ahoj - Hi , Diky - Thankyou (this is informal but much more easy to say than the formal way....., Prosim Please......

Hello /Hi = Nazdar/ Ahoj (nazdar / ahoy)
Good Morning = Dobre rano (dob-reh rahno)
Good Afternoon = Dobre odpoledne (dobreh otpoledneh)/ Dobre den (dobreh den)
Good Evening = Dobry vecer (dobree vecher)
Good Night = Dobrou noc (dobroh nots)
Goodbye = Na shledanou (nas-khledanoh)

My name is = Jmenuji se (Yeme-noo-yih-seh)
Nice to meet you = Tesi me (tye-shee myeh)
Do you speak English = Mluvite anglicky? (mlooveeteh anglitskih)
Where are the restrooms = Kde je zachod (gdeh yeh zah-khot)
Where is the nearest telephone = Kde je nejjblizsi telefon (gdeh yeh neye blishee telfon)
How much is it? = Kolik stoji (kolik sto-yee)

Thank you = Dekuji (dyejkooy-yeih) or Diky
Please = Prosim (proseem)
Sorry = Pardon /
Yes = Ano (ano')
No = Ne (neh)

I don't understand = Nerozumim (nerozoomeem)

Here are your phrases and the phonetic pronunciation:

Jmenuji se... ('men-OO-yih seh) My name is....
Tesi me. (TYEHSH-ee myeh) Pleased to meet you.
Mluvite anglicky? (MLOO-vee-teh AHNG-leets-kee)
Do you speak English?
Kde je tu nejblizsi telefonni budka?
(KDEH jeh too NEH-bleezh-shee teh-leh-FOH-nyih
BOOD-kah)
Where is the nearest phone booth?
Kde jsou prosim toalety? (KDEH sow PROH-seem TOH-ah-leht-tee) -- Where are the restrooms?
Dekuji (DYEH-koo-yee) Thank you.

Czech pronunciation tends to be on the first syllable of a word.
You may want to get the reference work which also contains a pronunciation cassette or CD. (see below) It has a lot of good tourist information about the Czech Republic as well as phrases for every imaginable situation.
