My daughter is nine years old, and I've lost her immunization records. Her school says they have no records(not really possible, they say they may have been lost?) and she hasn't had to get any in about 4-5 yrs, so the dr. office shes been going to has no records. How do I get ahold of her immunization records? Anyone else experienced something similar and has advice, I'd greatly appreciate it. thanks and God Bless!

go to the doctor or health clinic where you got her shots they will have a copy. And they may ask for ID.

Has she ever had to go to the health department for immunizations? They will have records there. Also, the doctors office should have records, it is part of her permanent medical record with them. Tell them you need a copy of her immunizations (some states require a certified copy) they have to give it to you

A lot of states have immunization records on line. Our state (Michigan) it is law to record these shots. Ask your health dept if they have that service. Your Doctor should have the records if not you will have to trace back to the dr. before that. Some one has to have them... Good luck

Go to the doctors where he got the shots . the school should never have had the originals , always give them a copy of anything that important . Good Luck

That's odd that your doctor doesn't have a record. The public health department in your area will have access to this info.

whoever gave her the shots has to keep a record and report it to the health department check both places now they do charge for another shot record card

Her dr. office has to have her records (the office where she recived her shots 4 yrs ago), if they don't have them - sorry, there's no way to look it up.

Oh, wait - maybe your insurance records from that time....??

Ask the Dr's office to order them from the archives. Thats what I had to do.
