Once you had it in the box you should have kept it in a cage, that wasnt very bright letting it go like that, so its gone dude it isnt going to trust you now and it may not trust anyone now. You should have never turned it loose again and expected it to come back because it wont now.

Buy it cockatiel food. Just go to like a Target or Wal*Mart, they have cockatiel food in bags, seeds on treat sticks and millet spray. My cockatiels love those. Keep him/her in a smaller cage and keep the cage near you as much as you can. Let him/her see you putting food and water in it's cage. If his/her wings are clipped you can put it on your shoulder. That's what I did with my male cockatiel, and now he's very friendly. If it's a female and gets used to you, she will never bite, at least not hard. But if it's a male and ever if he gets used to you, he will still bite you pretty hard if you try to pick him up and he doesn't want you to, but be friendly at all other times.

Make sure it's comfortable with you. Make sure it associates positive things with you. GIve it its favorite food a lot. Don't punish it or make it afraid of you. Try to avoid eye contact too.

Don't try to spend too much time with it at once. Slowly and steadily increase the time you spend with it until it gets more comfortable.

part of your question sounds like you care for the bird,
the additional part I would wonder if the bird would be better
if someone else would take care of it.

Follow the vet's advise. Be loving, kind and gentle.

good luck

It takes time to make the bird trust you. give it love and care and it will come around. maybe it had a previous owner that wasn't very nice to it.
