Don't write her a note, you'll confuse her. Ask some of her friends or just walk up to her and introduce yourself in a polite way and then you can start a conversation which will lead on to the things you really want to know.

Find out your targets' name (let's suggest Tina for this example). Get a friend to call your work phone asking to speak to his girlfriend, Tina. Approach Tina, saying you have "a call for a Tina from her boyfriend and is that her?", hoping she says she doesn't have one!
