Basically, Molten rock rises from the deep within the earth. Then, it penetrates the crust. It may collect in large pockets in its path. When enough hot stuff accumulates, the very high pressures will force it out to the surface. This is the eruption, and may range from barely noticeable to extremely violent.

It depends. Basaltic volcanoes erupt mildly and rhyolitic volcanoes can erupt very explosively. Other volcanoes erupt in between these two extremes.
