PCP is a Primary care physician. that's the doctor you go to when you get sick. you pick one person that you are always going to see and your insurance company registers that as your PCP.

The Provider is the doctor/clinician/hospital etc.

If you need to be referred to another doctor, like a specialist, your primary doctor will give you a referral to someone. EX: if you have uncontrolled high blood bressure he might send you to a cardiologist.

I hope I answered your questions. someone in the benefits department where you work should be able to explain the insurance program to you if it's through work. Otherwise, call the insurance company and ask them.
