Yes but most of the time the only reason it would get stuck far enough where it's nearly impossible to reach is because it was forced behind the pubic bone(due to sexual intercourse) So make sure you don't forget to remove the tampon beforehand.

yes, if a man is penetrating and pushes up further inside. Normal insertion usually will not cause it to get stuck. Once it is stuck and unable to come it will cause a nasty odor and infection. Which leads to a trip to the doctor to have it remove.

It can get stuck because of dryness or crooked insertion, but you will always be able to retrieve it. Relax, squat on the floor, insert your fingers and feel for the string while bearing down.

Yes it can. I knew someone that had sex with a tampon in and had to go to the doctor to get it removed.

Yes, but not permanently. Sometimes they get stuck because to large (absorbent) a size is being used and there is not enough liquid to facilitate removal.
