Have you tried doing something different? Change your routine - you might start to see some change. The body gets used to doing the same thing, and settles into the routine. Mix it up a bit. Good luck.

Make it a habit to change your routine every 8 weeks. You don't have to completely change the exercises, but change the order you do them, try changing the body parts ie, instead of back and biceps, maybe for a few weeks do triceps and biceps together. You get the picture...
Also, diet is key. Make sure you are consuming enough protein, drinking enough water, and getting plenty of sleep. If you track your calories, then mix that up a bit too. For instance, if you consume 1800 calories a day, then one day consume 2000 calories, and the next consume 1600. Your metabolism will keep guessing and be more active.
All of the other answers here are very good (at least, the ones I have seen). Read them all, you will benefit from the advice.

Your probably at a plateau. This normally happens after you have been working out for awhile. For example on bench it might be around or just above your weight. Its not anything that is set in stone or always happens at a certain weight but it is something your body does. Just take on more weight and push through it.

You may want to change your routine (sequence of exercises) and you may also want to consider doing different exercises per muscle group (incline bench press instead of regular bench press).
Also, what are you eating. Try to eat low fat, no sugar, high protein and moderate carb meals. You may also want to try eating more times per day (5-6) and make sure the meals are small, portion size meals and drink a lot of water. As always though, make sure you check with your doctor before changing things up. Hope that helps.

Because you see yourself every day, you don't notice the change. As you improve, your rate of improvement slows down. Just be patient and continue; the results will come.
