Absolutely, if they need it. It is also proper to escort the handicapped person inside the bathroom & inside the stall if necessary. You do what you gotta. Miss Manners used to write that the real essence of manners is not hurting the other person's feelings. So, are you embarrassing the handicapped person, or are you quietly assisting while allowing that person to retain their full dignity?

It depends on the person. I know plenty of handicapped people who would get very angry with someone for helping them, as they like the feeling of being able to do things for themselves still and not have to rely on someone else.
But if they ask for help, then it is perfectly okay. And if they don't ask, ask them if they would like some assistance. They will either say yes or no, and hopefully they will be grateful that someone is out there willing to lend a hand.

Depends on the disability. For some people in electric chairs it might be a wise idea as not all the doors are wide enough to fit larger chairs and you might have to help them transfer to the toilet. But basicly it's up to the disabled person, if they want your help they'll ask for it.

Only if they ask and then you wait outside until they need your assistance to leave. In this day and age all premises are supposed to be accesible (In UK DDA)

you need to ask first. just because they have a disability doesn't mean they cannot do for themselves. everyone deserves the dignity to take care of themselves as much as possible.
