Between 6/12/06 and 6/16/06 were you anywhere getting some sort of treatment or seeing a doctor? If not then call BCBS and ask them what the charges were for(doctor, place, etc.). If you still don't recognize the charge then tell BCBS it was a mistake and to investigate the charges. Clerks in offices do make mistakes and the wrong person can get charged for something.
