I think it means watch what you're saying or doing before you regret it or something like that. I heard that phrase on an episode of Roseanne.

It means that if you don't take a look at the things you are doing. you will undoubtedly end up with some sort of repercussions.
i.e. a coworker decided to steal my ink pen, I told him that I knew he had stole it and the he should be careful about who he steals from.
