The workout videos are difficult for most people to keep up with.
The best thing to do is to get a meal plan from a registered dietician. You can be referred to a dietician directly from your family doctor. By following the meal plan for awhile, you can get down to a weight where it is safe for you to exercise. Your doctor can tell you exactly what weight that is, once they take into account any other health problems that may be in existence. Don't forget, your doctor is not there to judge you, and in all likelihood went into the profession to help people, so do not be intimidated.
Congratulations on taking the first step to better health: you WANT to lose weight, and are exploring your options. That is the first step and getting started is the hardest, but after that, you will begin to feel better.

One step at a time. The person may have difficult working out, but walking every day for about 35+ minutes with diet will improve the person's stamina, and soon enough the person will be able to step up the workout. I would also consult with a doctor before starting any workout routine.

pick up a 5 or 10 pound weight at the gym and visualize that weight coming off holding the weight in your hands helps bring home just how heavy even 5 pounds of extra fat can be

a gastric bypass, to reduce the stomach size, then cardio and A LOT OR weight training, to burn calories, fat & tone up the body
when the ideal weight is archieved then that person can included pilates or yoga to cardio & weight training
a gastric bypass is not a must but he/she must find a way to control the consumption of food,...4-5 meals per day in small sizes helps

Try walking back and forth in your living room. Susan Powter did that when she was overweight and started seeing the weight fall off. You have to eat right to of course.
