Write it yourself and use personal, unique moments in his life that you wish to remind him of... Have him read it aloud, listen closely to his voice, and count how many times he clears his throat...

The best one would be one that you wrote yourself. Don't worry about writing a "good" poem. Just write from the heart. What does he mean to you? Dads and moms don't care how well you write, they love to have things from your heart.

i a published poet i got a dad one but i dont think itll work so let me tweek it for ya..hold on...
You are the best dad anyone could ever ask for,
when we had so little, you always made it more,
your in our hearts threw out every day,
you are so full of life always wanting to play,
for this we love u with all are might,
to bad your now to old to go out n fight,
your gettin older yet you still look young,
maybe next yr ill have to give ur old butt a lung.
all jokes aside, your still the best,
no one can say your not better than the rest.
