Required by who? And as applied to whom?
Very different rules apply, for example, to military handling of the flag, versus government official, versus civilians.
Title 4 of the US Codes sets for the preferred etiquette for handling the US flag. But those rules are not legally binding in and of themselves against civilians, because they are phrased as preferences and suggestions ("should"), and not as requirements ("shall", "must").

Nothing is technically required. The "rules," however, include not allowing the flag to touch the ground, not displaying it below another flag (except the UN flag), displaying it properly (the field must be at the upper left corner if hung vertically), not leaving it out in the rain unless it's rainproof, illuminating it if it's flying at night, folding it properly (it should be a triangle), and retiring it if it's damaged.
