Go to the IRS website at http://www.irs.gov and type in "SS-4". You can download the form from the IRS website, print it out, fill it in and mail it to the appropriate IRS office. They used to let you call in for it, but I think that service was shelved a while back.

Good luck on your new business! As a helpful hint, you may want to look into retaining an accountant. She could help you with the process of starting a business, because you're probably also going to need to register with your state (unemployment tax number, sales tax registration, etc.) and you may need some advice as to how to set up your business-sole proprietorship, partnership, corporation, S-corporation, LLC, LLP, whatever. Do this before you even open the doors. A good accountant can be your new best friend when you decide to open your own business.

It depends if you need the number for yourself or your business.

If it's for your business you can apply for an Employer ID Number or (even if you don't actually employ anyone, yes it's confusing). You can now apply form SS-4 on-line, instructions here: http://www.irs.gov/businesses/small/article/0,,id=102767,00.html

If you are ineligible for a Social Security number you need a Taxpayer ID Number or TIN as an individual you file form W-7. You need to take the form http://www.irs.gov/pub/irs-pdf/fw7.pdf and proof of IS with you to an IRS center. Instructions are included with the form.
