They come for the special surgeries because America has the best doctors. However, when comparing healtcare policies, it's not the rest of the world vs. the U.S. because then the U.S. wins. Comparisons should be made between comparable situations; hence, U.S. and Western Europe and Canada.
In those countries, everyone gets healthcare but each country has a different way of doing it. Some countries have private healthcare too, some have better records, and some have worse.
The problem for many people is why the world's wealthiest country does not have universal healthcare for it's citizens, when it actually spends more per person than the other countries anyway.
I think the best system would be like one in France, where every person has basic and free healthcare, yet there is a vibrant and strong private business of hospitals and healthcare. This way the people go to the hospital for preventative and healing care, but have the option to go somewhere else. As far as waiting for so long, that is a dubious myth conjured up by HMO's and politicians in this country; people who have a financial incentive to lie. People who think that the market will solve everything and people who run that market and don't want to lose business.
The French system should be looked at and improved and I think all Americans should have healthcare and also the option of private doctors; preventative measures will actually lessen our costs in the future.

People come here for certain specialties which are available that are better here than elsewhere; burn treatment is one of these. People also go to other countries where there a specialists in other fields. Generally, only wealthy people or those who can get sponsorship make these trips, and they get a lot of press because they involve unusual or severe situations. Movie stars in the US have traveled to Switzerland and Mexico to get special treatments there.

When it is said that other country's health care is better that is referring to the average healthcare, not the unusual specialties. The medical profession uses two indicators for the effectiveness of a country's health care: infant mortality and life expenctancy. These are quantitative measures, and based on them, the US is far from the best.
