The left ring finger is traditional as in ancient times it was believed that a vein from that finger ran directly to the heart. And, yes, once the wedding ring is put on that finger, the engagement ring goes on after. Some cultures also don't use wedding rings at all. In parts of India the bride has a special necklace tied on by the groom during the wedding ceremony "tying the tali" and that is worn in lieu of a wedding ring.

Left hand, 3rd finger (not counting the thumb as a finger), or the finger just next to your pinkie. When you get married, people generally wear their wedding band below their engagement ring on the same finger, since the wedding band will be closer to the heart.

In the States it's the left hand but I know in many European countries the ring goes on the left before the wedding and then on the right after.

If there are severe burns to the left ring finger, or if it is missing, it is acceptable and customary for the lucky lady to wear her engagement ring on the right ring finger.

It goes on your left ring finger, then after you get married, if you choose to wear both, you put your wedding ring on first, then your engagement ring next. This way, your wedding ring is "closer to your heart".
