I can only tell you what would happen in New Hampshire. Every state is different. And in truth, having someone get treatment against their will should be difficult. You are talking about having a portion of his human rights taken away.
In general, treatment can be court ordered if the person is ruled to present a danger to himself or to others. The standards for proof can vary greatly. Contact your local mental health center to get the rules for having someone hospitalized or treated under court order. It's possible that the cat abuse is sufficient, but it may not be.
Since he is also 78 years old, it's possible that the area agency for the elderly may be of some assistance.
All of these courses will require a lot from you or your friend's family members. However, it sounds like there are enough reasons to be concerned that at a minimum having a doctor monitor his mental status would be reasonable.

Tell him he's going somewhere really nice that you think he'd like to go. e.g. out for dinner, bowling, for a walk in the park. You have to decide on that one. Then take him to the Dr. Or have a Dr. do a home visit. Maybe a visiting nurse could assess him first. Check your local hospital and ask about community nurse visits. Other than that call 911 and ask them to pick up this poor gentleman who could benefit from psychiatric evaluation and medication by the sound of it. Thanks for caring!!!

There is a department of elderly affairs if u call them they can give you numbers of doctors who can come to assess him. Many doctors do house calls and can refer you to places where they can probably move him in to. He is going to need a stable environment and a med adjustment. The man should be at home anymore not only is he a risk to everyone in his path but to himself.

call social services.
that's probably your best bet... since he is a threat to his own health, as well as your/whoever is helping him.
if he's driving, you can tell social services the details & they will make him take a driving test... that should be interesting, but that would do the trick.
