Talk with your nearest Army recruiter, and tell them you are prior service. Bring your DD-214 with you to prove it. If the break in service is less than 2 years then you don't have to go through Basic Training (or at least that was the rule in 1988), but you are stuck with your previous MOS. The Army used to have an age limit of 30, but they have increased it to 40.
When I re-entered the Army the recruiters weren't that happy with me, and they processed the prior service people last. They also sent me overseas for 3 years on my first assignment. First time recruits usually get their first duty station in the US. When you go with the recruiter pack up all the pieces of your uniforms that still fit. You will be issued with standard orders that allow you to ship goods, but you need another person to do it and they need a Power of Attorney, you can even ship your car. (Use a Limited Power of Attorney that you write yourself, and get it Notarized. Include a clause that says the Power of Attorney expires after a time interval, or upon delivery of a written document stating the Power of Attorney has expired. You can cancel a Power of Attorney at any time, but one that has a short time limit is the best.)
