Can adult adhd affect a relationship even when the person is medicated?

Yes. My husband has adhd and is medicated. And sometimes his adhd affects me. He is a wonderful husband but he doesn't take his medication the way that he should. All week long he takes too much of his medication making him unable to sleep. Then when he gets to the weekend he doesn't take his meds and he crashes and sleeps the entire weekend. I'm trying to get him to take his medication on a regular schedule and I worry about him when he does this. So yes, it does affect the relationship.

As someone who has ADHD, I can say that medication is not a cure-all. I still have trouble with certain attention issues, which can give my wife fits. All the medication does is settles me down and gives me the ability to focus a bit better, but I still have to learn how to use this ability.

Possibly if the person with ADHD has not learned to treat a woman nicely.

There should not be any if they have understanding and intelligence.
