Tiger,Phil and Jack spoke about it, but never named the Player, who stated that he " was trying somethign new", when asked about using the illegal driver on tour. Whoever he was, he apparantly was having success with that driver and ultimately his success stopped or slowed after he stopped using it!!

Neither Hank nor Greg were ever cited for using an illegal driver. 100% conjecture at this time. Some player have hit illegal driver outside competition including TW. Palmer advocated the ERC club that was illegal but no one to date has been sanctioned for using an illegal driver.
There was an illegal driver in Colin Montgomery's bag a while back but that was because it was a fifteenth club not because the driver itself was illegal.

I think there were multiple players, but notably, Hank Kuehne used one.

Also, Woods is sure that many more have used or are using one without anyone knowing. The PGA has implemented a testing procedure, but the only time they will test is if someone voluntarily wants their driver to be tested, and that defeats the point. It's not like the "cheaters" will submit to being tested. However, the PGA uses an honor system.
