Well, my migraines were caused from a clotting disorder called Antiphospholipid Antibody Syndrome. Not saying this is what you have but I know I was miserable for many years because of them.
First know your triggers and avoid them. Take preventative medications such as antidepressants, anti seizure. And try abortive medications, if not contraindicated, to stop the migraine in its track. For more information, please see the links I have below.

darldm1 has a great response for you. Vasospasms in the brain are what cause the pain. Typical over the counter help includes aspirin, tylenol, and caffiene. That doesn't work for everyone. It's what is in typical 'first try' stuff. There are a lot of things a doctor will try once he/she gets a chance to look you over and talk about your symptoms. I would encourage this right away. They are very debilitating and you know how painful they are. Some treatments include drugs to stop them like Imitrex. There are some heart medications that are used to try and prevent them like calcium channel blockers.
I had them at two times in my life. Once as a teenager. They went away after a few years then they came back again in my 30's. Went away again. Some where really bad requiring Demerol and Phenegran. I was completely non-functional for a day or two. A lot has changed and improved since my last attack. Imitrex came along right when I quit having them. What a disappointment...if it were only around sooner!
Best advice: Got to your regular doctor and ask about seeing a neurologist that specializes in migraines. Someone already suggested that, I know. That is going to be your best bet. Hope this helps. Jeff the nurse in Nashville.

There are numerous theories on what causes migraines. One prevalent theory is that the arteries in the brain contract and sensing this the brain causes the body to release chemicals which result in the arteries expanding. The arteries expand into tissue thus causing the pain. That is probably why your headaches get worse when you smoke. Nicotine is a known agent which causes arteries to go into spasms. There are a lot of meds available for migraines. Some are anti-seizure meds. Some of the newer migraine medications work by constricting the arteries. Of course these meds must be avoided if you have a heart condition. The only sure way of finding the correct medication is by trial and error as not all medications work on all people. Have your primary care physician schedule you to see a neurologist who is conversant in migraines. You may also try a headache clinic if there is one near you. For most of us it is just a case of finding meds which at least control the severity of the headache. Also laying in a darkened quiet room can help. I know for myself when I get a bad one I go to my room take a pain killer and try to sleep. Hopefully when I awake the episode has passed.

There has been allot of promising research on migraine headaches and there some new medications out on the market in the USA to help curve and stop the pain from a migraine. You should do some research on the web and see if those medications are available where you live.. I sure hope this helps..Good Luck..

Fluid rehydration
Some medications if you are not allergic to them:
aspirin (beware that aspirin will worsen things if you actually have a bleed in your brain - make sure you have a CT scan showing no bleed before you try this)
stemetil (prochlorperazine)
largactil (chlorpromazine)
Usually when I am treating a patient with migraine, if it is a clear cut migraine, I will put an IV cannula in and run 1000mL of normal saline ... give IV prochlorperazine 12.5mg (adult dose)
After a little bit of time I will give them 2-3 dissolved aspirin
If all this doesn't work 12.5-25mg IV chlorpromazine is often helpful.
