The short answer:

The H1B starts with an employer in the US filing a petition for that particular worker. If the petition is approved, the worker then applies for the visa at a US Embassy or Consulate. There is - of course - an interview with a consular officer to assure that the petition is genuine and that the worker has the qualifications for the job. It all seems okay, the visa is issued.
