Is there a difference in waiting time if you have an appointment? How does the line system at the DMV work?

Appointments make a difference! Everyone gets a number, different letters are for different things.
A- reg app; B- non app reg; C- express item non-app; J- disabled window; F- DL app; G non app DL.
Monday, always bad, they get all the weekend impounds and DUI's; Tuesday is better if you can wait, but don't get a late fee, they already get enough of our money.

Yes, there is a difference with an appointment (in California). If the wait time without an appointment was an hour, with an appointment it would be around 15 minutes. Although it can vary greatly, this is just an average.

And Brandywine's description of the ticket system is accurate. So based on your needs, you can be directed to the correct technician who (hopefully) knows the most about the topic you need help with. But Mondays and Fridays are bad. After a holiday is really bad. Last couple days of the month and first few days of the month are really bad.

With the DMV, it is always better to go in the middle of the month, middle of the week, middle of the day. I have much quicker turn around when I do that. Also, if your DMV has online services, use them! They are a life saver.
