emergency guys, like firemen, dont worry about locks, or doors. They usually know how to smash or break a door quickly, and if that is too difficult ( steel security door etc. ) and takes too much time, they just walk thru a wall or other shortcut. Most apts and houses
have many thin plaster walls you can kick thru, faster than a door.

Anytime that there is reasonable cause to believe that a crime is occuring, or that a person is endangered, the Police have the ability to 'break in' to the location.
In the case of a fire, the Fire Department not only can enter the house, but may also 'negate' any vehicle parked in front of a hydrant. Normally this is done my breaking the windows and feeding the line through the car.
If Paramedics can not access a patient, they will normally have a police officer respond (often times Police are the first people on scene regardless).

They may have tools to open the lock or they'd take the doorknob off with just an ordinary screwdriver. Otherwise, they would just kick the door in or, if there's a window in the door, break the window and reach in and unlock the door from the inside. In the event of an emergency it wouldn't be very hard, especially for firefighters as they have many different pieces of equipment.

If some situations, like a suicide attempt, drug overdose, or other possible illegal activity, the paramedics will call the police, and they can kick the door in. In other situations a rescue truck or fire truck will be called, and they will open the door either by kicking it in or taking an axe to it.

They have the right to break in the house. They carry a tool that will get them in if that's their only way in.
