Arnold-Chiari defect is where the back of the brain (where it meets the spinal cord, and usually also involving the cerebellum) is pulled downward towards the bottom of the skull and thus distorted. It often accompanies spina bifida, but may also be a part of other syndromes. It may cause no particular problems and be an incidental finding on examination, or it may result in things like blockage of the flow of fluid out of the brain, balance problems from cerebellar involvement, etc.
