In the UK, if you are off work, you just need to tell your employer when you go back and as long as you were off for less than five days, you can "self-certificate", which means the employer has to fill out a form, but you don't need to do anything else. Any longer than that and you need to go to your GP to get a sick note. I don't know what happens for self-employed people though, or what you do if you ARE the employer!

Little confused about what you want to certify yourself for but if you mean you've had a few day off work due to sickness then I THINK you can legally self certify for three days, I know some organisations allow five days. For this you do not need to obtain any certificate, your employer should have the paperwork and should conduct a return to work interview with you. For longer periods of absence I think you would have to see a doctor who would, if he/she agreed you were unwell, write you a sick note. I don't know how this works for the self employed.

If your in the UK then you can self-certificate your self from work for up to 5 days. You then have to go to the docs to get signed off for any longer.
In theory you dont need to a certificate to do this. Just tell your boss you have been/ will be ill.
But in practice most people prefer to get a certificate.
You should be able to pick one up from the receptionist at your doc surgery.
Although if you boss gets funny about these things I would go to your Docs and get him/her to sign you off in the first place.
Hope this helps and hope you get well soon!
