I got a letter from my daughter's school that majorly lacked detail?

if you would like your child to participate please fill this out and sign:
and after that it said name, address and phone number, and a place for me to sign and date.

Do i send a note asking for more detail or do I call and ask?

Here in Georgia, the ASP is for students who may have trouble meeting the standards set forth by the gov't. It's a very helpful program as it has assisted my teenage daughter in getting more knowledgeable about her courses.
If your child needs help, fill it out and return it.
Oh, and daughter loves it!

I would ask for more detail. For all you know it can be someone who is trying to hurt you or your daughter. There isn't enough info for anything to be done. where in the emergency numbers, the fathers name all the things that is on a regular form?

the after school program is also a safe place for kids to go til their parents get off work call the school on monday for more info or check the schools website

after school program is where your kid stays after school and gets help in things they are lacking or with homework. but they didnt tell you what time it starts and ends??? looks like they need to hire someone who knows what to do lol.
