When studying, eliminate distractions like TV, radio, music, etc.
Get adequate sleep. Get enough exercise. Eat well.
If this doesn't work, see your doctor. You may had ADD (attention deficit disorder) or OCD (obsessive compulsive disorder) and there are meds for that.
No shame in needing them. Sometimes the brain doesn't get the chemical it needs, just like a diabetic doesn't get the insulin he needs. No difference.

You need to try and relax. It sounds as if you are under a lot of stress right now. If this gets work, you might want to talk to someone about it, but until then:
Only good things will happen. You will read and understand what you are reading. You will learn it. You are relaxed and ready to learn. You will sleep well at night and remember what you have learned during the day. Bad thoughts will not enter your mind. Everything will be just fine. It will all work out and be OK. Enjoy yourself. Take a little time for a walk in fresh air. Everything will be just fine.

Moutain Dew... The regular stuff perferably... I too have the problem... I recently diagnosed myself with A.D.D. because i can not stay doing one thing and focused without think about 10 different other things.... One way that does and has helped me in school is taking notes... Even when reading and not in class read a paragraph to yourself look at what may be on a test or if there is anything important in that paragraph *( go sentence by sentence if you have too )*... then write it down.... make sure you aren't doing this at 2 in the morning... and before you start reading... If something big has happened like you just found out your sister sally just had a baby... deal with that first... if there are things that are causing you think when doing your homework then think about your problems and try to adjust those first... talk to a room mate... or write a in a journal, or just think... keep in mind your brain can only do so much at once...

Remove distractions. Watch less TV and play fewer video games. They alter your brain to expect a lot of stimulation with little effort.
I think your brian comes up with these ideas being you are a creative person. You need to train your brain to use that creativity at the appropriate time and allow you to concentrate at other times.
When reading, pick an uncomfortable chair so you don't fall asleep. Focus on the book and don't allow your mind to wonder. Think about the characters, events and locations instead. Go to the library to read if you cannot find another quiet location.

You can start by accepting the fact, that we out here cannot help you. You must help yourself.
Start by taking control. This exercise will give you command over your mind and conscious will and intent.
1. Start by placing a pen or pencil in front of you on the table.
2. Tell yourself to pick up the object. (Pick it up)
3. Tell yourself to put the object down. (Put it down)
4. Tell yourself to repeat exercise 10 times. (Repeat 10 times.)
5. Upon completion, tell yourself that you are in control of your mind, thoughts and actions.
Now do some Brain Writing.
1. Take the pen or pencil used before and start writing positive affirmations. ie, "I am in total control of what I think", write this a number of times that you choose. 10 times, 100 times. your choice.
2. Then write, "The sun is warm, the grass is green and I have no room for negative thoughts."
3. Be creative, write what you want to happen in your life. Concentration, positive thoughts, you name it.
When you get a negative thought, immediately stop and mentally say to yourself, "Cancel, Cancel, negative thoughts."
Relax, close your eyes and count slowly from 1 to 10.
Then count from 10 to 1. When you feel your mind starting to drift, stop, say to yourself, Focus, Focus, then clear your mind and continue.
Your problem, that can be corrected, is that you know very little discipline in your life.
Take the time to reward yourself when you do something good and make sure you also tell yourself that you desere the reward.
When you study, and you find your mind drifting, stop, clear your mind by saying focus, focus or cancel, cancel, negative thoughts. and continue by backing up to the last place you left off. Learn to re-read what you have just read, and you will find there were places you drifted and didn't catch it. Then re-read.
I could go on and on, but I have given you the basic tools to vastly improve control of your thought process.
You can train your mind and if you perform the above on a daily basis, everytime you get a chance, you will achieve control over not only your mind, but your whole life and your future as well.
There are numerous subliminal programs available that deal with concentration, focus and mental discipline. Search on eBay.
Learn to control your mind, and you shall control your life.

Very few can concentrate will if they are being overwhelmed with what sounds like negative things going on in their life. First you need to find way to lessen negative things going on in your life or learn how to adjust to them. That would be most import thing to do in the long run. while your working on that will offer some short time solutions. One they call clearing your mind. Take a deep breath. Slowly let pass through lips shaped like you going to whistle. NOT TO OBVIOUS AND DON'T WHISTLE. While the air is passing through you lips mentally think I'm blowing out those bad thoughts. Take another deep breath do same thing with lips and as you feel your lungs relax gentle say to your self relax. Take another deep breath and as you do it and feel lung rel ex say relax to your self. take another deep breath and as it passes your lips think how all those bad thoughts are gone.
and not coming back. If your not feeling dizzy do one more and say relax. Here comes important part try and concentrate on what the book is saying let each sentence fill your mind.
Remember this is only short term fix will you get help with the big stuff.

First off, I have always found it helpful to take multi-vitamin B every day. It helps calm nerves and makes it easier to focus on things I need to learn.
Next, make sure you are getting enough good sleep.
It is good to have a quiet spot to study. If you can make a place where all you do there is study, you can mentally leave your worries outside that place. Maybe you need to go to a public library and ask for a quiet study spot to get you away from distractions around the house.
I believe since you recognize what is holding you back, you have the power to overcome it.

Go for meditation in a well ventilated room before you start reading. Try to control your mind by concentrating on a particular object, sound, vision, theme etc. When you are thorough with it, apply the same method for education also. Ofcourse, it will take sometime to organize but it is the most effective way, I believe.

You sound like you may suffer from some type of ADHD, possibly inattentive type. Check out some online web surveys to see if you fit in any of their categories - you don't necessarily need drugs to fix the problem but a good therapist can help you with concentration techniques. There are also alot of co-disorders that go hand-in-hand with ADHD and may cause your brain to come to almost a complete stop when you need it to be working the most.

Develop a schedule. Write in detail all activities for the day and stick with it. Try to specify your goals and accomplish it. Forego any activities that are not congruent with you goals. If there are unsettled issues going through your mind, you can allot a portion of your daily schedule to deal with it so that when you sit in your study table, you're at peace with yourself. Or if you want, write your negative thoughts in a piece of paper and throw it away or burn it (as if your thrashing away all those negative energy). Convince yourself that good energies abound in us. Try watching the radiant sunrise, feel the warmth of the air, appreciate the movements of the clouds, the lush vegetations, the comic character of people... always see the brighter side of life!
Going to the church, prayer or meditation also helps!
