It's been such a laugh seeing Americans trying to answer this, usually wrongly!
As an English person, I can say with full authority that the word 'wanker' is an insult. It is derived from the slang word 'to ****' meaning to masturbate (usually applied to a male), however it is not normally used literally.
Most of the time it is a mild insult, or, in the right context, can be used enderingly. On a scale of 1-5, with 5 being the rudest, 'wanker' is around the 3-4 mark.
