Multimeters are a cheaper than most electical mistakes.

Use a digital multimeter to see which way the polarity is. Usually, the white striped wire is the positive.

It depends on the adapter, there is not a standard to which they are all made.
Usually it states the polarity on the side somewhere, if not you could simply use a voltage meter and test it yourself.

I have hundreds of adapters.

There are no RULES at all, and any manufacturer can use any colour code or wire coding to connect any plug. Really. Don't trust
any "rule". You have to have a meter to really see. Or, a diode and a bulb, etc.

sorry, but I have seen too many different reversals to trust any general theory... they all have to be checked...

there should be a marking identifying which is positive and which is negative it is always in the schematic drawing of every adapter

If its painted with a black strip on a red background, its negative and other one with complete red is positive
