In the Air Force we call them name tapes.

There are many sites that will do it and most local taylor/alteration shops can do it also.

Here is a site: http://www.1800nametape.com/

If you know someone in the military, try going to a used uniforms shop. They have all of these old name patches, and if you have a common last night, it's bound to be there. It's really nothing much. It's just simple sewing machine-made embroidery on a piece of material.

If you live anywhere near a base, go to a dry cleaner/tailor shop that works with military uniforms. A lot of them can order or make nametapes for you.
If you dont live near a base/post, try one of the websites other people listed.

You can order from Brigade Quartermasters by phone (Toll-free) 1-800-338-4327 with a credit card... they make them for all current and past uniforms, all services. Dog tags, too. Their web site is www.actiongear.com.
