Since I don't know where you live, I can only give general answers....

First look in the phone book under "Stained Glass Design" or "Glass, stained and leaded glass artists or designers". They are probably going to be the most expensive way to go, but they're the easiest to find. If there aren't any stained glass stores call a glass and mirror store, they'll refer you in the right direction.

Another approach would be to call around (community colleges, crafts stores, stained glass stores that offer classes, and even local art galleries) to see if there is a journeyman level glass artist who could do it for you. someone who has experience but isn't all the way professional can do just as nice of work but for less money. Also, often the teachers of glass classes charge less for commisioned work.

Finally, you might go to a craft fair and see if you can find someone selling stained glass there. Often they'll take on commission pieces too.

A few notes to help you in your design. if you go to a stained glass website or store and look through their pattern books, you'll find there are a lot of pretty music themed patterns. if you already have the design picked out (or design it yourself) that will reduce the cost of the work. and of course, the simpler the design, the cheaper the finished product will be (i.e. fewer pieces of glass).

in terms of installing it, some glass artists know how to, but you might end up having to get a window installer to help you. Please, if you don't want to have to remove and repair these windows (and especially if they're going to be on the ground level) install a plate of clear glass on the outside of the stained glass piece so that birds and things flying up from your lawnmower, etc don't damage the window. I don't know exactly how to do this so that it doesn't fog up but i'm sure you can research that.

I would post on Craig's List. You can find all sort of interesting folks there. Or head out to a local Saturday Art Market and talk to the artisans there. It won't be totally cheap but I bet it will look cool.

i had a front door with an ugly glass insert. i went to an art fare in town and found someone who desighed stained glass. they came over, measured, let me pick the design and viola it was done.

Call your local stained glass supplier/store. Many stores do custom work and also teach classes so maybe you could do your own.
