how do I sign up for child health plus for my son? it would be part B.?

I have the application, but I'm not clear who to send it to for part B and if I have to send copies of paystubs too or not?

You should really go to your local Health Plus office, thats what i did. But you are deff. going to need to send pay stubs regardless and also your sons info. Social, birth certificate etc..
