Be honest and polite. Tell him that you value his friendship & would never want to lose him. And as his friend you notice that he has not the best smelling breath. A breath issue.
I would go and look up solutions online so you can offer advice. He may or may not be ware of his breath so don't make it a personal attack.
He might pull awat at first because he's embarrased. Don't take it personally & stay available. He'll come back around.

Pull him aside one day and simply tell him. There are many causes of halitosis (scientific name for bad breath). The most common is obviously bad oral hygiene, but the second to that is the decay of teeth. He may not realize this is a problem and if so, he will certainly be appreciative of your concern. Tell him in the context of concern and all should be well. Beaware that his problem could be of dental decay and one that he may not be able to address because of financial issues. Try to help him get a handle on this by suggesting that you will be there for him to provide a gentle nudge (or secrect word or phase) to tell him that his breath is out of control and he could then take a mint or two.. or three.
Tell him with concern, respect and the desire to help and I'm sure he wil appreciate your being honest.

tell him how you feel about his breath he may feel bad but you have your own right as a human to like and dislike things so if you don't feel comfortable with how his breath smells tell him that he breath smells bad because it is your life and if you will have to sit beside him all through the year then you will have to tell him what you think about him and that may make him brush his teeth even if he fills bad or maybe he doesn't know that he has a horrible breath then may be apologize

I'm going to have to change my usual approach to the problem after this; generally I'm with the 'kick him in the nuts' guys... I have to admit that it hasn't had the best result.

That's a tough one. When I was a kid I had a girl friend with a knock out bod and bad breath. I hoped her breath would get better but no matter what I tried, it never got better.
I dropped her.
