I have lived in Eastern Tennessee and Western NC, and it is a common synonym for yell, scream, etc.

It is also used to mean "give me a call" whether in person or by phone. Such as, "If you need anything, holler at me."

holler can mean screaming and crying to the top or ur lungs when its in the progressive hollering or past tense she holler my name so loud. i dont know where u heard it but bahamians say holler a lot. we almost never say y u yelling at it. we say y u hollering at me

In Georgia it tends to be way overused. We even use it as a word for whisper,secretly. "Let me holler at you for a min.." Then pull them off to the side.

I don't hear it too often in the east, but my daughter is always claiming we are hollering at her. hee hee.

I don't know about widespread... but it is a word... and not a hick word either...
