The number is really not reliable above 160. Yes, Marilyn von Vos Savant did game the system, and took it repeatedly to achieve a score, which last I heard was 238.
The IQ doesn't really change, but the measuring system is flawed. My IQ was tested repeatedly when I was in Grad School, as a part time job to improve the tests. Lowest I ever scored was 160, highest was 210. That is based on a flaw in the system.
If you consistently score over 135 on the S-B test, you are a legitimate genius, which means you score higher on the standardized tests than 98 percent of the population. The 98 percent number is the real qualifier, because there are so many types of IQ tests.
Many people have been more intelligent than Marilyn. IQ generally measures three forms of intelligence, when there are at least 11 identifiable forms of intelligence.

I have read about a woman named "Marylin vos Savant" who has an IQ of 220 plus. I don't know the exact number but it is up there.
