You should have a reference number on it, list why the application for credit was disapproved, and the credit bureau that you used.
Dear Mr. Bad Credit:
In review of your credit application, we regret to inform you that you request for credit has been denied. The reasons for denial are:
Derogatory credit file or public judgments
Plain bad credit.
The credit agency we used was Bad Credit Bureau and there address is 123 Bad ST, Badlands, CA 12345.
