Regularly irregular - rhythms where the 'background' heart rate is regular but there are occasional extra beats or missed beats. For example venticular ectopics would cause extra beats, some forms of heart block will caused missed beats.
Irregularly iregular - there is no regularity to the heart rate with varying beat to beat intervals at all times - the commonest example of this is atrial fibrillation where the natural pacemaker of the heart is not functioning and the ventricles respond to irregular electrical stimuli from above. Try and look at some ECG rhythm strips to see the effect.

A 'regularly irregular' pulse still follows a pattern - for example it may consistently drop every fourth beat. In some cases this may be normal - for example some athletes demonstrate a periodic dropped pulse at rest. At a minimum, it suggests there is still a working mechanism governing heart function.

An 'irregularly irregular' pulse varies in rate or period in no discernible pattern. This is almost always a bad thing.

If a pulse of say extra systole occurs every five normal pulse waves it is regularly irreglar.If the irregularity ie changed pulse w (change can be in rate, pattern or volume)occurs sometimes after two beats, and after sometime five beats, and still some other time after eight beats, it is said to be irregularly irregular. Sinus arrhythmia caused by inspiration and expiration is difference in rate during inspiration(slow) and expiration(fast) and is thus regularly irregular. Arrhythmia(lack of rhythm) in cases of atrial fibrillation is irregularly irregular. In cardiac tamponade of constrictive pericarditis the pulse is paradoxical in that it increases during inspiration and decreases during expiration.In thyrotoxicosis pulse rate is same both while breathing in or out. In anxiety(panic attacks) pulse is irregularly irregular but this irregularity disappears wth exercise. Irregularly irregular pulse of atrial fibrillation is aggravated by exercise.

A regularly irregular pulse always beats irregular

but an irregulary irregular beats off sometimes, such as menopause moments, but not always

A pulse that is regularly irregular forms a predictable pattern. One that is irregularly irregular cannot be predicted with any degree of accuracy.

regularly irregular means that does not happen, for example, once every 10 seconds, changes by 2 seconds each time, in a set direction, irregularly irregular means that it changes different amounts in different directions.
