Generally it's just an expression meaning to turn someone on. In practice you never want to blow directly in the ear. Personally I'm more of a fan of side or back of the neck.

Where I come from, it's part of an expression: "Blow in my ear, I like a bit of passion while I'm being screwed."

It means "I think you're messing me about."
